Shaping the future of health care
Every physician at Kaiser Permanente is vital in helping lead the way to a better future for health care. This strong commitment to our patient’s health has earned us Patient-Centered Medical Home designations, in recognition of our integrated system and the physicians who are setting the standard for improving primary care.
A member-focused system of care
Within our exclusive contract with the largest nonprofit health plan in the U.S., Kaiser Foundation Health Plan, Inc., we are responsible for providing all medical care to KP members in Colorado. Our partnership enables us to closely integrate primary, secondary and hospital care, so we can provide high quality, affordable and comprehensive health care to our members.
As part of a multispecialty physician group, our physicians have the luxury of having Kaiser Permanente’s network of medical offices, support staff, and services at their fingertips. This team approach allows physicians to delegate care to appropriate support staff enabling them to concentrate on direct patient care; develop better relationships with patients; and spend less time on clerical work. We offer a wide-range of team resources often not available in a typical primary care setting, including: asthma care managers, senior care coordinators, diabetes care managers, perinatal home health practitioners, wellness specialists/dietitians, clinical pharmacists, and more.
Staying connected with technology
The most comprehensive and advanced health information system, HealthConnect®, keeps the nation’s 8.6 million members and health care teams connected, coordinated and current. This high-tech communication enables us to bridge the gaps often seen in traditional care delivery systems.
From Colorado, a national impact
Both KP and CPMG have an extremely strong commitment to evidence-based medicine, life-long learning, preventive medicine, and influencing health care policy and reform. Through our integrated health care delivery system and our clinicians, we are transforming health care.
